All new clients wishing to book a session are required to fill out the following form. If you are requesting a couples session, each person must fill out the form individually.
***Established clients may contact me directly to book. Established clients do not need to fill out the form again.
If this form is not filled out correctly, it will not be taken as a serious inquiry.
Discretion is essential to all parties involved. Privacy is honored. Information is properly discarded after screening.
Patience is appreciated. I am not always near my phone or computer. Evenings and weekends can take longer for reply.
